Parto gemellare
Il parto gemellare è, come facilmente si può intuire, quello che porta alla nascita di due o più gemelli. Di solito, le gravidanze gemellari sono più brevi di quelle normali, durano in media 37 settimane, fino ad un massimo di 38, dopo le quali si programma un cesareo o si induce il parto. Questo perché la placenta, sia doppia che unita, invecchia più velocemente. Di conseguenza, i bambini frutto di una gravidanza gemellare di solito sono più piccoli di quelli nati da una gravidanza singola. Il parto gemellare può essere naturale, per via vaginale, o più frequentemente cesareo. In caso di parto gemellare naturale, di solito la durata è la stessa di quello singolo, in quanto solo la fase espulsiva dura qualche minuto in più (di solito i gemelli nascono a 4-5 minuti di distanza).
Parto gemellare naturale
Nei parti gemellari spesso si ricorre al taglio cesareo, ma il parto naturale non è da escludersi in ogni caso. Se i bambini sono entrambi in posizione cefalica (cioè con la testa rivolta verso il canale vaginale), il parto vaginale è possibile. La posizione dei bambini, però, non è l’unico elemento da prendere in considerazione. La mamma, infatti, deve essere in perfetta salute e il suo bacino abbastanza ampio da permettere la fuoriuscita dei piccoli. Ma come nascono i gemelli? In modo molto semplice: il primo bimbo normalmente, proprio come se fosse solo. Per quanto riguarda il fratello, se è in posizione cefalica uscirà senza difficoltà dopo qualche minuto, se invece è podalico il medico ne cercherà i piedini per tirarlo fuori. Se infine è messo di traverso, con una manovra di rivolgimento verrà girato in posizione podalica e fatto uscire. Infine, in alcuni casi la nascita del secondo gemello può essere talmente difficoltosa da richiedere un cesareo.
Parto gemellare cesareo
Il parto cesareo in caso di gravidanza gemellare è sicuramente più frequente rispetto a quello naturale. In particolare, si ricorre al cesareo se:
- entrambi i bambini sono in posizione podalica o di traverso;
- vi è un ritardo della crescita o una forte differenza di peso tra l’uno e l’altro;
- la placenta è invecchiata (placenta previa);
- le sacche amniotiche sono condivise in una gravidanza monoamniotica monocoriale, cosa che potrebbe provocare un attorcigliamento dei cordoni ombelicali;
- i gemelli sono più di due;
- i gemelli sono monozigoti, perché si corre il rischio che si verifichi una trasfusione feto fetale, nella quale i vasi sanguigni alimentano uno dei bambini più dell’altro, che cresce meno.
Parto gemellare record
Ultimamente si sente sempre più spesso parlare di parti plurigemellari, di tre o più gemelli, la cui causa sono quasi sempre cure ormonali per la fertilità. È questo il caso di parto gemellare record la cui storia ha fatto il giro del mondo, ovvero quello di Nadia Suleman, trentenne originaria di Fullerton in California, che nel 2009 ha dato alla luce ben 8 gemelli. In realtà la donna, che aveva già avuto 6 figli, tutti attaverso la fecondazione assistita, pensava di aspettare 7 gemelli, ma al momento del parto ha scoperto che c’era un bimbo in più! Nadia durante il parto è stata assisitita da un’equipe composta da 46 persone, tra medici e infermieri. Ovviamente il suo è stato un parto cesareo, programmato addirittura nove settimane prima della scadenza dei termini della gravidanza. Per questo motivo i neonati erano tutti molto piccoli, con un peso che andava dagli 800 grami ad un kilo e mezzo.
