Parto indotto: che cos’è, quando farlo, tecniche e rischi

Il parto indotto è una procedura medica attraverso la quale viene stimolato il travaglio del parto naturale. Viene effettuato solo se strettamente necessario, in quanto non è privo di rischi. Di solito si sceglie di indurre il parto quando  il travaglio non comincia spontaneamente, come ultima spiaggia prima di praticare un taglio cesareo. L’induzione del parto può avvenire attraverso diverse tecniche, come il gel di prostaglandine, la fettuccia o manovre manuali. Vediamo quali sono i casi in cui il parto può essere indotto, quali sono le tecniche migliori e quali i possibili rischi a cui la mamma e il bambino vanno incontro.

Parto indotto: quando farlo

Il motivo più frequente per cui si sceglie di indurre il parto è il superamento dei termini della gravidanza. Il bambino dovrebbe nascere  tra la quarantesima e la quarantaduesima settimana, per cui se si superano questi termini e il travaglio non comincia spontaneamente, prima del cesareo si ricorre al parto indotto. Altri motivi che possono portare a questa scelta sono:

  • rottura delle acque senza le contrazioni del travaglio o in anticipo rispetto alla scadenza dei termini della gravidanza,
  • poco liquido nel sacco amniotico (oligoidramnios),
  • infezione dell’utero,
  • placenta deteriorata,
  • distacco della placenta,
  • leggera sofferenza fetale,
  • anomalie cardiache del bambino,
  • patologie della madre come ipertensione, colestasi gravidica e diabete.

Il parto può essere indotto con diverse tecniche, come la somministrazione di ossitocina, l’uso della fettuccia, del palloncino o del gel e il distacco manuale delle membrane. Vediamo i dettagli:

  • parto indotto con gel di prostaglandine: all’interno dell’utero vengono introdotte delle candelette vaginali a base di prostaglandine, che ammorbidiscono la cervice, agevolandone la dilatazione;
  • parto indotto con fettuccia: si inserisce in vagina una piccola fettuccia di garza imbevuta di prostaglandine, lasciandovela per massimo 24 ore. In questo modo il collo dell’utero si dilata e si induce il travaglio;
  • scollamento delle membrane:  questa tecnica prevede il distacco delle membrane fetali dall’utero per indurre il travaglio. Può essere fatta manualmente con un dito, oppure attraverso l’utilizzo di un palloncino che viene inserito nell’utero e gonfiato fino a raggiunge circa 5 cm di diametro. Di solito è abbastanza dolorosa;
  • ossitocina somministrata per via endovenosa, che stimola le contrazioni del travaglio.

Parto indotto rischi

Indurre il parto non è una pratica priva di rischi. Innanzitutto, sappiate che il parto indotto è più doloroso del parto naturale. Questo perché quando il travaglio è spontaneo, il nostro organismo produce endorfine che attenuano il dolore, cosa che non avviene se le contrazioni vengono provocate artificialmente. Altri rischi sono:

  • necessità di ricorrere al parto cesareo,
  • riduzione del ritmo cardiaco del bambino,
  • nascita prematura,
  • prolasso del cordone ombelicale,
  • infezioni sia nella madre sia nel bimbo,
  • rottura dell’utero,
  • emorragie post partum.

A volte il parto indotto può non andare a buon fine. Di solito, dopo un primo tentativo si prova con un secondo, prima di ricorrere al cesareo. Il secondo tentativo viene fatto solo se il parto non è urgente, se la salute della mamma e del bambino non è a rischio, se non è stato già provocato lo scollamento delle membrane e se non si sono rotte le acque.
